A dedicated Care Manager works closely with you, your MSU Health Care provider, and your loved ones to ensure you're getting the attention you need.
You don't need to be a technology expert — our devices are really easy to use. When you receive your Care Everyday devices, take them out of the box, put the batteries in (they’re included), and turn them on. It’s that simple.
An entire medical team is there to support you and will collaborate with your current team of MSU Health Care providers. Your health is monitored around the clock, giving you peace of mind.
You will participate in a 15-30 minute telehealth video visit with a Care Everyday physician who gets to know you, determines your eligibility, reviews your medical history, and creates your personalized care plan.
Care Everyday sends you a set of easy-to-use health devices that measure and automatically report on your blood pressure, blood glucose and weight. Your health information is automatically sent to a Care Manager who reviews your data every day. The devices are easy to use and don’t require WiFi. Many patients qualify to receive all this at no out-of-pocket cost.
Your dedicated Care Manager works with you to find ways to improve your health. If your health numbers are of concern, your Care Manager will check in with you to discuss next steps. You can always contact your Care Manager by text, phone, or video chat.
